Application Process

We’re thrilled you’re considering Family Life Academy! Choosing a school is about more than academics – it’s about finding a community that partners with you in raising children who love God, pursue learning with joy, and step into life with confidence. 

At FLA, admission is more than a paperwork process – it’s the beginning of a partnership. We welcome families who believe education is a privilege, not a right, and who desire to join a Christ-centered community shaped by Biblical truth. 

Step 1: Complete Required Forms

Our online application is just the first step. Please download and complete the required enrollment forms below to submit your child’s application.

Step 2: Submit Your Application

  • Complete our online application below.
  • Pay the non-refundable $75 application fee (per student). 
  • Submit required documents.

Step 3: Schedule a Family Interview

Once your application is received, our admissions team will contact you to arrange a family interview. This is an opportunity for us to get to know your family and for you to ask questions about our mission, values, and expectations.

Step 4: Student Review

We may request a placement assessment and/or review previous school records to ensure we can provide the best support for your child’s learning needs.

Step 5: Admissions Decision

Applications are processed in the order they’re received. Siblings of current students are given priority if class space is limited. After review, families will be notified of the admissions decision.

Step 6: Enrollment

Once accepted, families will: 

  • Complete enrollment forms 
  • Sign the Parent & Student Handbook Agreement 
  • Submit health/emergency forms 
  • Pay the registration fee to secure placement 

Application Checklist

Kindergarten/New Student

  • Enrollment Application (one per child)
  • Emergency Blue Card (one per family)
  • Signed Handbook Form
  • Copy of Birth Certificate
  • Copy of Shot Record / Signed Exemption Form – There are required boosters before a child can enter Kindergarten
  • Registration Fee ($75 for one student or $100 for two or more students)

First-Fifth Grade

  • Enrollment Application (one per child)
  • Emergency Blue Card (one per family)
  • Signed Handbook Form

Sixth Grade

  • Enrollment Application (one per child)
  • Emergency Blue Card (one per family)
  • Signed Handbook Form
  • UPDATED Shot record / Signed Exemption Form (Boosters are due when child turns 11)

Seventh-Eighth Grade

  • Enrollment Application (one per child)
  • Emergency Blue Card (one per family)
  • Signed Handbook Form
